We welcome any patient living within our Practice area (please see map). As a new patient, we invite you to have a New Patient check with the Practice Nurse within Three months of joining the Practice.
A map of our practice catchment area is shown below. If you live in the practice area and would like to apply to register with us please telephone or call at reception to discuss making a registration appointment. When patients are applying to join the practice list we do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
Delays in the registration process can be avoided if you have your NHS number with you at the time of this appointment.
You should also bring your passport and proof of address i.e. council tax statement, utility bill, etc. All newly registered adult patients will be asked to complete a questionnaire and have a consultation with our Healthcare Assistant or Practice Nurse within two weeks of registering. Patients on regular medication will need.
